Each state has different requirements when it comes to registering your new equipment. The information below will provide what you may need and who you will need to contact. It is the clinic’s responsibility to meet the State regulations before the installation and submitting the proper forms after the installation.
Before we install your new equipment, please ensure you have the following in place
- Your clinic is registered with your State to take radiographs or use of lasers
- You have the proper lead shielding in place according to your State regulations
- A scatter report has been completed by a physicist according to your State regulations (needed for radiographic equipment)
Please note that the links below may change as they are maintained by the individual states. Sound does it’s best to keep these links updated but we are not able to catch every change. Please let us know of any changes at installations@soundvet.com.
Some states require that you submit a physicist and/or shielding report. Please check your state requirements for any trusted individuals that can perform these duties. The State requires you to submit all information prior to installation, including serial numbers. Sound cannot provide serial numbers until the day of installation. Please note on the sheet these will be provided after the installation as the machine is built to order.
Shielding Plan: Required - expand for more information
The State of Alabama requires that a shielding plan be approved by the Office of Radiation Control. There are no specific minimum lead or drywall requirements because the amount of shielding required is based on the room layout and equipment use.

To open a new facility or add an additional x-ray machine at a Veterinary facility in the State of Maine, each owner of the x-ray unit must:
- Have a shielding design (a drawing of the purposed x-ray facility indicating what shielding material is required) performed by a Qualified Expert, If this is a replacement x-ray unit used in the same facility in the same room, please fill out the form HHE804A and get it signed by your Qualified Expert.
- Submit the shielding design to the Radiation Control Program for review. Send to Radiation.DHHS@Maine.gov
- Complete and submit to the Radiation Control Program the registration form with the proper registration fee. Use form HHE 804 for fee submission and form HHE 805 for (each) x-ray machine data.
- Have the x-ray equipment inspected by a Qualified Expert within 30 days of installation and every 5 years. A copy of this report must be sent to the Radiation Control Program, any areas of non-compliance must be repaired/adjusted within 60 days and a copy of the service representative’s report sent to the Radiation Control Program (Radiation.DHHS@maine.gov).

Minnesota
IMPORTANT: The Facility must be registered before installation. See veterinary guidelines. An appropriate shielding plan must also be completed before construction and use. Handheld devices are not allowed in Minnesota. If you are using a handled device, you must fill out a Variance form prior to purchasing one and get approval prior to the first use of hand-held x-ray equipment.
PLEASE NOTE that EVERY x-ray, handheld, mobile, or stationary must have an FDA and EPE form filled out If you have purchased an equine generator, please contact your local x-ray company to perform an Equipment Performance Evaluation. Sound as an installer/seller must inform the customer that the unit will need inspection (EPE) by MDH approved inspector for the XRAY unit.
Installation calibration and equipment performance is required to be done by the service provider for Minnesota: For portable XRAY Units, we cannot just drop ship them to the customer, like many of the other states. We will need someone that is registered with MDH to do the actual calibration at installation at the customer site. The customer needs this information to provide to the state when they do their variance to have a hand-held unit. Also, this will need to be repeated, based on the frequency in the MDH schedule.

Steps to Ensure Your Facility and Equipment Are Registered

- STEP 1 Shielding Plan: The registrant should contact service provider who is registered with Radiation Protection Section (RPS) to perform and submit a shielding plan review to this agency prior to construction and or structural modifications and prior to installation of equipment. Contact a registered service provider with Class III or Class IV under the service provider’s listing and the appropriate category – medical, dental, veterinary, etc. depending on the type of practice and X-ray equipment. Link to Service Provider Listing: Class III, IV, and V. The service provider will submit the shielding plan to the N.C. Radiation Protection Section for review. *Note: Steps 1 and 2, Shielding plan and acknowledgement are not required for industrial applications, bone density units and *standard mammography units. (Shielding plans are still required for stereotactic, 3D and tomosynthesis mammography units)
- STEP 2 Acknowledgement Letter: Once a shielding plan review is submitted to the agency by the service provider, the shielding plan will be reviewed and evaluated and a letter of acknowledgment will be mailed to the registrant. Instructions regarding any actions needed by the registrant after installation of the X-ray equipment will be included on page 2 of the acknowledgment letter. (* see note in step 1)
- STEP 3 Equipment Installation: After the registrant receives an acknowledgement letter from the agency, contact a service provider that is registered with the agency for installation of equipment. Contact a service provider that has a “Class II” under their listing and the appropriate category – medical, dental, etc. depending on the type of practice and X-ray equipment. The registrant should provide a copy of the acknowledgement letter and shielding plan review to the installer. The equipment must be installed exactly as projected on the acknowledged shielding plan.
- STEP 4 Post-Installation Survey: (required on all installations): The registrant must schedule to have a postinstallation radiation survey with a service provider who is registered with this agency. The survey must be performed within 30 days following initial operation of the unit(s). Contact a service provider that has a “Class V” under their listing and the appropriate category – medical, dental, veterinary, etc.

- STEP 6 Report of Installation: The service provider performing the installation of the X-ray equipment must complete a federal Form FDA 2579 “Report of Assembly” for each unit installed. A copy of this report must be provided to the registrant, the FDA, and the Radiation Protection within 15 days of completion of the installation. Note: Equipment exempt from the federal Form FDA 2579 such as Industrial, analytical and some veterinary, the service provider is still required to report the installation to this agency on the agency form. - STEP 7 Radiation Safety Program: Registrant must designate a Radiation Safety Officer for their facility and develop a Radiation Safety Program. - STEP 9 Personnel Dosimetry: All registrants must provide personnel dosimetry for each X-ray operator. The service provider providing personnel dosimetry services must be registered with this agency. The registrant should look for a service provider with a “Class VIII” under their listing. - STEP 10 Registration Updates: Registrants are required to submit updates when any change renders the information contained in the application for registration or notice of registration no longer accurate. A change of ownership, facility or billing contact must be reported on the application for registration. Relocation of a registrant, equipment, new installations, X-ray component (control panel only) changes or a change in the status of equipment use such as active or not in use, sold or donated must be reported. Registration updates require signature of an authorized person and date.
- STEP 11 Safety Culture: Registrants are required to be knowledgeable of rules and regulations regarding radiation safety. State Registration Forms
Before we can schedule an installation, the State of South Carolina requires the following items below. We have provided links and forms for your clinic registration and shielding reports.
- The facility must submit a Facility Registration Approval (FRA) application to the Department for review. Upon approval, the Department will provide the facility with an FRA letter containing the facility’s Registration Number.
- The facility’s shielding vendor must submit a shielding plan application to the Department for review. Upon acceptance of the shielding plan, the Department will provide the shielding vendor with a shielding acceptance letter containing the assigned Log Number. (The shielding plan application and the FRA application may be submitted simultaneously, or the FRA may be submitted first, not vice versa.) The equipment may not be installed prior to receiving that Log Number.
- An Approval Letter of your Shielding Report
- The Shielding Report requires a Physicist visit.
- The location of the x-ray CANNOT be changed once the approval letter is accepted
- Upon completing the installation, the installation vendor will complete the FDA 2579 and submit a copy to the Department.
- The facility must register the equipment within 30 days.
- An Equipment Performance Test must be performed on the unit prior to use.

Specific Information**PLEASE READ**
Facility Registration: (Guide B9)
FACILITY REGISTRATION APPROVAL (See RHB 2.4) Prior to installing an x-ray machine, a facility (Sound Customer) must apply to the Department for a Facility Registration Approval (FRA). To receive a Facility Registration Approval, complete and return the FRA request form DHEC 0845 along with the non-refundable application fee of $62.50. A FACILITY SHALL NOT INSTALL OR CAUSE TO BE INSTALLED AN X-RAY PRODUCING MACHINE UNTIL THE DEPARTMENT HAS ISSUED A FACILITY REGISTRATION APPROVAL. If a facility moves to a new location, a letter must be submitted to the Department stating the new location address and any updated facility contact information. Facility Registration Approval is not transferable to a new owner or any additional locations. A new Facility Registration Approval and processing fees are required for the acquisition of an existing facility.
Quality Assurance and Equipment Performance Test (EPT) (Guide B9)
QUALITY ASSURANCE (See RHB 4.2.16) A quality assurance program should include equipment performance tests (initial or annual calibrations) of the x-ray system and repeat analysis. It is each registrant’s (customers) responsibility to evaluate the performance of their xray imaging systems and tailor their quality assurance plan accordingly. 1) Equipment Performance Tests (Calibrations) Equipment performance testing for each veterinary x-ray unit must be performed, by a registered vendor, at the time of installation, every five years, and at any time the Department deems necessary. Equipment performance test results must include numerical data. It must also include an indication of Pass/Fail or Compliant/Non-compliant. Any items found to be non-compliant must be corrected within 60 days of receiving the report. Records showing the equipment performance testing and the correction of any non-compliant items found must be retained for five years or until the next Departmental inspection, whichever is later.
Shielding Plans (Guide B9)
SHIELDING PLANS (See RHB 4.4) Before construction, a facility is required to submit a radiation shielding plan and a shielding review fee to the Department for review and acceptance. The shielding plan must be reviewed by a Class III, IV, VII, or IX vendor. After the equipment has been installed, “as-built” drawings and an area survey (if applicable) are required to be submitted. The shielding plan must be accompanied by a $62.50 Shielding Plan Review fee. 4 A shielding plan is required in the following situations: 1) An existing x-ray machine or control generator is replaced with a unit that has increased capabilities; 2) Any mobile or portable units are used in a single location for more than 5 consecutive days; 3) Any space is utilized as a radiation area for more than 5 consecutive days. A shielding plan is not required upon the replacement of x-ray equipment with like equipment and when there are no other changes that would render the original shielding plan inaccurate. This must be determined by a Class III, IV, V, VII, VIII, or IX vendor. The vendor must notify the Department on DHEC Form 2779. No fee is required for the submission of an equipment notification form. This notification must be submitted to the Department prior to the replacement.

Contact the state in order to facilitate their registration. If you are removing an x-ray machine, the CUSTOMER must fill out and submit the TN-RHR Deletion Form to the state and keep a copy for your inspector.
The registrant must notify the State of TN of all changes per TN regulations. It is the responsibility of the registrant to notify TN according to SRPAR which requires that the Division be notified of any changes in location, ownership, or possession of any x-ray producing machines within 10 days of the change. This includes the permanent discontinuation of use or transfer of all x-ray producing machines at a facility. Without this notification, the registration is still considered active, and subject to registration, which can result in registration fee’s being assessed according to SRPAR Rule 0400-20-10-.24(2).

Laser In accordance with rule 289.301(g)(1)(A) in addition to the requirements of subsection (g)(1) which is on pages 301-8 and 301-9, each person having a Class 3b or 4 laser for use on animals , shall submit an application to the agency within 30 days after beginning operation of the laser if it’s not mobile. Mobile is if the laser travels outside the facility. A certificate of registration with mobile authorization must be received before the operation begins.
